All requested information must be provided before submitting form.
After submitting form, the order will not be completed until payment for tags has been received Payment must be received within ten (10) days or tags will not be ordered. After ten days, an additional $25 will be required to complete the validation. There is also a $25 returned check fee. - After the deadline which will be posted on this website, late tag orders will be $15/tag
